The organization now knows where work breaks down, but still runs on informal, undocumented, or inconsistent practice that varies by person, shift, and residence.
The work CCO performs
CCO converts diagnostic findings into clear workflows, documentation standards, responsibility structures, escalation pathways, review cadences, follow-up mechanisms, management controls, and implementation measures.
What "redesigned" actually means
A process is not redesigned when it has merely been documented. It is redesigned when responsibility, timing, escalation, evidence, and closure are all defined.
