Phase 01 · The Purpose

Diagnosis creates operational truth.

Before anything is redesigned or installed, leadership needs an accurate, shared account of how the organization actually operates — not how it is assumed to operate. Fragmented organizations rarely lack effort; they lack a single, evidence-based picture of where work breaks down.

The condition entering this phase

Repeated incidents, inconsistent documentation, unclear follow-up, dependency on one or two key managers, communication loss between shifts, a growing compliance-preparation burden, and different operating practices between residences.

The work CCO performs

CCO examines how information moves through the organization, where responsibility becomes unclear, and where operational events fail to become management action — the exact points at which daily activity stops turning into decisions.

Leadership leaves the phase able to see the operation clearly and decide, with evidence, what must be restructured first and in what sequence.

How it connects to Redesign

The findings set the redesign agenda. Phase 02 converts the prioritized problems into workflows, standards, and accountability structures instead of starting from assumptions.
